• 以下の条件を満たしている方につきましては、当所でお手続き（再交付）が可能です。
• Reissuance procedures can be carried out at this office for individuals who meet any of the following conditions:

▶証明書の有無にかかわらず、10年以内に仙台検疫所（巡回診療を含む）で接種した方

▶Individuals who received vaccination at the Sendai Quarantine Station (including outreach clinics) within the past 10 years, regardless of whether the certificate is currently in their possession

▶接種した時期にかかわらず、仙台検疫所（巡回診療を含む）で接種しており証明書が手元にある方

▶Individuals who received vaccination at the Sendai Quarantine Station (including outreach clinics) and still have their certificate, regardless of when the vaccination was received

• 申請の前に必ずお電話にてご相談ください。
• Please contact us by telephone before submitting an application.
• 原則窓口での申請及び受け取りとなります。
• In principle, both application and collection must be completed at the service counter.
• 再交付希望とお伝えください。
• Please indicate that your inquiry is regarding reissuance.
• 再交付には１～２週間程度かかりますので早めにご連絡ください。
• Reissuance typically takes approximately 1–2 weeks. Please contact us as early as possible.

• Available at the service counter.

②収入印紙：880円

②Revenue Stamp: 880 yen

• 事前に郵便局等でご購入の上、持参してください。
• Please purchase the revenue stamp in advance at a post office or other authorized retailer.
• 収入印紙はおつりが出ませんので、金額をそろえて購入してください。
• No change will be given for revenue stamps; please ensure that the exact amount is purchased.
• 880円を超える場合は「黄熱予防接種手数料の超過納付書」が必要です。（窓口にてご用意しております。）
• If the amount exceeds 880 yen, an Overpayment Form for the Yellow Fever Vaccination Fee is required. (This form is available at the service counter.)

③パスポート（原本）

Passport (Original)

• 本人確認及び証明書の記載内容確認のため必要です。パスポートの原本をお持ちでない方は、顔写真付きの身分証をお持ちください。
• The original passport is required for identity verification and to confirm the information to be recorded on the certificate. If you do not have your original passport, please bring an alternative form of photo identification.
• 婚姻などで氏名が変更になった場合は、新旧のパスポートの写しなど、変更になったことがわかるものをご用意ください。
• If your name has changed due to marriage or other reasons, please provide documents that clearly show the change, such as copies of both the previous and current passports.

※現在所持している方のみお持ちください、新しい証明書と交換になります。

※Please bring this only if you have it; it will be exchanged for a new certificate.

申請に必要な物①～④を持参の上、来所してください。

事情があり窓口での対応が難しい場合は、検疫所までご相談ください。

If it is difficult to apply at the service counter due to special circumstances, please contact the Quarantine Station.
