・時間厳守でお願いします。時間に遅れた場合、接種できない可能性があります。
・Please arrive on time. Late arrival may result in being unable to receive the vaccination.
予約方法
How to Make a Reservation
予約受付は仙台検疫所仙台空港検疫所支所で実施しております。
Reservations are accepted at the Sendai Airport Branch Office of the Sendai Quarantine Station.

受付時間：平日 9:00～16:00（土日祝日・年末年始除く）
Reception Hours: Weekdays, 9:00–16:00 (Closed on Saturdays, Sundays, national holidays, and during the year-end and New Year holidays)
受付開始：接種日1カ月前 （土日祝日・年末年始の場合は翌開庁日）
Reservations Open: One month prior to the vaccination date (If this falls on a Saturday, Sunday, national holiday, or during the year-end and New Year holidays, reservations will open on the next business day.)
受付締切：希望接種日の前週の水曜日（定員になり次第、予約受付終了）
Reservation Deadline: By Wednesday of the week preceding the desired vaccination date (Reservations will be closed once capacity is reached.)
・事前予約制としております。
・Advance reservations are required.
・電話予約は、日本語や聴力に不自由のある方を除き、本人に限ります。
・Telephone reservations should be made by the applicant.Exceptions apply for individuals who require assistance due to hearing impairment or language difficulties.
・母国語が日本語ではない方は、原則として医療通訳が必要です。
・In principle, applicants whose native language is not Japanese are required to be accompanied by a medical interpreter.
・18歳未満の方は保護者の方がご予約下さい。
・For individuals under 18 years of age, reservations must be made by a parent or legal guardian.
・出発日まで十分に余裕を持ってお申し込みください。
・Please apply well in advance of your departure date to allow sufficient time.
黄熱予防接種について
Yellow Fever Vaccination
・黄熱流行国への渡航や入国時に黄熱の予防接種国際証明書（イエローカード）の提示が求められる国へ渡航される方などが対象となります。
・This vaccination is intended for individuals traveling to countries where yellow fever is endemic, or to countries that require an International Certificate of Vaccination or Prophylaxis (ICVP) (Yellow Card) upon entry.
・各国の入国におけるイエローカードの提示に関する個別の条件等は、渡航の際に関係する国の大使館へお問い合わせください。
・Entry requirements regarding the presentation of the Yellow Card vary by country. Please contact the relevant embassy prior to travel for further details.
・イエローカードは、接種後10日目より生涯有効です。
・The Yellow Card becomes valid 10 days after vaccination and remains valid for life.
・料金は収入印紙で17,680円分です。（国際証明書発行手数料880円を含む）
・The fee is 17,680 yen, payable by revenue stamp. (This amount includes an international certificate issuance fee of 880 yen.)
・次のいずれかに該当すると認められた場合は接種をすることができません。
・Individuals Not Eligible for Vaccination
▶ １歳未満の乳児
▶ Infants under 1 year of age
▶ 明らかに免疫機能に異常がある人、および免疫を抑える治療を受けている人
▶ Individuals with clearly impaired immune function, or those undergoing immunosuppressive treatment
▶ 明らかに発熱（通常３７.５℃以上）している人
▶ Individuals with a clear fever (generally 37.5°C or higher)
▶ 重篤な急性疾患にかかっている人
▶ Individuals with a severe acute illness
▶ 過去にこのワクチンに含まれている成分でアナフィラキシーをおこしたことがある人
▶ Individuals with a history of anaphylaxis caused by any component contained in this vaccine
▶ 胸腺に関連した疾患（重症筋無力症、胸腺腫）にかかったことがある人、および胸腺摘除術を受けた人
▶ Individuals with a history of thymus-related disorders (e.g., myasthenia gravis or thymoma), or those who have undergone thymectomy
▶ 上記以外に医師が予防接種を行うことが不適当な状態にあると判断した人
▶ Individuals deemed by a physician to be in a condition that makes vaccination inappropriate for reasons other than those listed above
2016年7月11日からイエローカードは生涯有効となりました。有効期限切れの証明書についても自動的に生涯有効となります。更新手続きは必要ありません。
Since July 11, 2016, the Yellow Card has been valid for life. Certificates that had previously expired have also automatically become valid for life. No renewal procedure is required.

接種当日について
On the Day of Vaccination
仙台医療センター1階総合受付にて午後1時45分（13：45）までに受付後、午後２時（14時）までに海外旅行外来受付へお越しください。
Please complete registration at the General Reception on the 1st floor of Sendai Medical Center by 1:45 p.m. (13:45), and then proceed to the Overseas Travel Clinic reception by 2:00 p.m. (14:00).
※時間厳守でお願いします。時間に遅れた場合、接種できない可能性があります。

黄熱ワクチン接種を希望される方
For Those Receiving the Yellow Fever Vaccine
➀収入印紙 17,680円分（国際証明書発行手数料 880円含む）
➀Revenue Stamp (17,680 yen) (This amount includes an international certificate issuance fee of 880 yen.)
- 事前に郵便局等でご購入の上、持参してください。
- Please purchase the revenue stamp in advance at a post office or other authorized outlet and bring it with you.
- 収入印紙はおつりが出ませんので、金額をそろえて購入してください。
- As change cannot be given for revenue stamps, please ensure that the exact amount is purchased.
- 17,680円を超える場合は「黄熱予防接種手数料の超過納付書」が必要です。（窓口にてご用意しております。）
- If the amount exceeds 17,680 yen, an Overpayment Form for the Yellow Fever Vaccination Fee is required. (This form is available at the service counter.)

- 本人確認及びイエローカードの記載内容確認のため必要です。パスポートの原本をお持ちでない方は、顔写真付きの身分証をお持ちください。
- The original passport is required for identity verification and to confirm the details on the Yellow Card. If you do not have your passport, please bring an alternative form of photo identification.

*If multiple doses are scheduled, the consent form must be completed in handwriting for each visit, and the original copy must be brought each time.

注意事項
Important Notes
- 黄熱予防接種の支払い方法は収入印紙です。（収入証紙や定額小為替ではありません）
- Payment for the yellow fever vaccination must be made by revenue stamp. (Revenue certificates or postal money orders are not accepted.)
- 接種会場は仙台医療センター です。こちらを参照ください。（仙台空港ではありません）
